What is this service?
The Lambeth stroke care advisor (LSCA) is part of the community stroke team and helps Lambeth residents who have had a stroke.
The LSCA can work with you to help you achieve the best possible outcome after your stroke, and can support you to access a variety of services in the community. The LSCA can also liaise with families, carers and health professionals involved in your care.
What areas can the LSCA help with?
Stroke can impact every area of life. The LSCA can help with accessing: · Befriending services · Transport · Peer support groups and activities (for example, social, artistic, or physical) · Financial support and benefits advice · Housing and utilities (such as gas, electricity) expenses advice · Courses, volunteering, and returning-to-work support services · Accessing carers, cleaning services and food shopping delivery · Health and mental wellbeing services · Support for your carers and family.
How can the LSCA help?
The LSCA can: · Visit you at home to explain the stroke care advisory service and discuss your goals · Put you in contact with local charities and groups · Direct you to other healthcare professionals · Help you communicate with services (through phone calls, letters and emails) · Refer you to helpful services with your consent, and contact them on your behalf.
The Lambeth Stroke Group
The LSCA also runs a monthly group. The Lambeth Stroke Group is held every second Monday of the month at the Aspire Wellbeing Gym (1 Park Hall Road, West Dulwich SE21 8EH) from 2-4pm. Please contact the advisor for more details.
