What is this service?
The Hospital 2 Home team is there to assist patients have a safe and comfortable discharge from Hospital. Our worker can either help escort you home or meet you there when you arrive. This is to ensure you are comfortable and settled upon your return.
They can also help with a number of other things, including:

Collect any items you need from home in the lead up to your discharge, this can include clothes or paperwork.

Ensure that there is enough food and other practical items in the house for your return.

Make sure that everything is in working order like the gas and electricity.

Assures minor adaptations and furniture is moved on the day of discharge and leaves you feeling safe and happy to be back home.
They can also link in with other Age UK Lambeth services to prevent future hospital readmission such as a cleaner who is part of the Hospital 2 Home team.
