What is this service?
A dynamic service which focuses on proactive support and crisis prevention. Helping the people we support to overcome complicated or recurring social care issues. Our care plans are person specific, following assessment and observation and our support approach empowers the individual to improve their quality of life by achieving meaningful outcomes, chosen by them. Community based 1:1 mentorship and support for adults living with a mental health condition, learning, neurodevelopmental disability or otherwise complex social care need.
